It complements rather than replaces medical care, home health, therapy or other licensed services.

What the family receives

Elder Ready provides written observations and prioritized recommendations based on the selected review and the information available. The report can support conversations among the family and the professionals already involved in their care.Additional components may include an Implementation Plan, Photo Documentation and appropriate educational resources.The report is observational and educational. It is not a diagnosis, clinical clearance or certification that a home is safe, and it does not replace medical care, home health, therapy or other licensed services.
